Complete an individual project sheet for each project that will change (increase or decrease) and for new projects. Change amounts to the actual amounts to be expended/drawn. |

This form lists the "old" amount, revised amount and the amount each project is increased or decreased. The total net change of all projects must equal zero.. |

If a new project is added, a project eliminated, or if the amount for any project is substantially changed, the revisions must be certified by the superintendent and board of education. |

Use this form to request authorization of State Bond Funds. Request the current or increased amount, not the total project amount. |

| State Bond Status (.pdf file*) | General report indicating the current status of the State Bond program. Includes statewide totals of bond funds drawn and total and completed projects. |
| Bond Funds Drawn (.pdf file*) | Summary of allocations, state bond funds drawn, balance, and percent drawn for each school system. |
